2008 3-A-Day™ of Dairy Vending Machine Grant Application

Dairy MAX, Inc. is awarding grants to school food service departments for the placement 3-A-Day™ of Dairy vending machines in high school and middle schools in 2008 calendar year.

Purpose:

The purpose of the grants is to help schools offer a healthy beverage and/or snack choices and to increase milk and dairy product consumption among teens during their peak bone building years.

Procedure:
  • The school food service director may apply for grants to purchase 3-A-Day™ vending machines.
  • Grants will be a maximum of $3,000 per campus.
  • Upon receipt of the application, a Dairy MAX representative will review the application and contact you to inform you whether or not you will receive the grant(s).
  • Campuses must have a minimum of 500 students. A review of the program will be conducted at the end of three months and schools will be asked for an average weekly sales report.
  • Funds can be used only to purchase a 3-A-Day™ of Dairy vending machine with the 3-A-Day™ logo or milk spot graphics.
  • Upon receipt of a copy of the school district’s purchase order or invoice, Dairy MAX will release the grant money to the school foodservice director.
  • The machine must be owned by the school for at least one year or the school agrees to return the grant.
Application Process:

Complete and submit the application form. A Dairy MAX representative will contact you. Funds are limited and grants will be awarded on a first come, first served basis.
